Is Ayushman Bharat Enough for You? What Kolkata Families Should Know

Is Ayushman Bharat enough for your family?
For everyday hospital needs, it is a real help. For a major surgery, a long illness, or a family that wants to choose its own hospital, it often isn't enough on its own. Think of it as a floor. It stops you from falling into debt, but it doesn't guarantee the care you would pick if money weren't a factor.
What Ayushman Bharat PM-JAY covers
Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) was launched on 23 September 2018. Eligible families get cashless cover of up to ₹5 lakh per family per year for secondary and tertiary hospitalization, with no premium.
Three things work in its favour:
It is free. You pay no premium, and treatment at an empaneled hospital is cashless.
It travels with you. A beneficiary can be treated at an empaneled hospital in another state, as long as the hospital and treatment are covered under PM-JAY.
It now includes senior citizens. Since September 2024, everyone aged 70 and above can be covered, regardless of income.
What changed in West Bengal
For years West Bengal stayed outside PM-JAY and ran its own Swasthya Sathi scheme. That changed in 2026. The state signed an MoU with the National Health Authority on 8 June 2026 and began the rollout that summer. Swasthya Sathi continues during the transition while beneficiaries are moved across.
This matters for Kolkata families because Swasthya Sathi was open to almost every resident, while PM-JAY is not. It works from the SECC 2011 database, so holding a Swasthya Sathi card does not automatically mean you are on the PM-JAY list. The state has announced the Mukhyamantri Swasthya Bima Yojana for residents who fall outside PM-JAY. Check the official portals for the latest rules, since details have been changing.
Where Ayushman Bharat can fall short
One shared limit. The ₹5 lakh is for the whole family for the whole year. Suppose one parent needs a cardiac procedure. That single admission can use up most of the limit, leaving little for a child's hospital stay later in the year.
Fixed package rates. PM-JAY pays hospitals set rates for each procedure. This keeps costs predictable, but it can limit your choice of room and, in some cases, of hospital.
Hospital access. You can only use empaneled hospitals. Bengal's empanelment is still being sorted out, so your usual hospital or doctor may not be on the list yet. Ask before you are admitted, not at the reception desk during an emergency.
Eligibility gaps. Lower-middle-class families who used Swasthya Sathi may not qualify under PM-JAY's poverty-linked list.
Hospital care only. It covers admissions. Regular doctor visits, tests and medicines outside hospital are not part of it.
Who is most likely to need more
Families who want treatment at a particular hospital, especially for heart care.
Households with elderly parents or a family member with a long-term condition.
Anyone who wants a private room.
Families who cannot risk one bad year using up the whole cover.
How a private policy fills the gap
A private policy sits on top of the government scheme rather than replacing it. Narayana Aditi is one option in Kolkata. It is sold by Narayana Health Insurance Ltd and offers up to ₹1 crore for surgeries and ₹5 lakh for medical management, with cashless treatment at Narayana Health network hospitals. In Kolkata and nearby districts there is also Aditi+, which adds top-up options and private room eligibility.
Your rights when you buy private cover
IRDAI's 2024 rules give you some useful protections:
The waiting period for pre-existing diseases is capped at 36 months.
After five years of continuous cover, an insurer cannot reject a claim for non-disclosure, except in cases of proven fraud.
There is no maximum entry age for buying health insurance.
Quick answers
Can I have Ayushman Bharat and a private policy together? Yes. PM-JAY is a government scheme, so having it does not stop you from buying a private policy.
Is my Swasthya Sathi card the same as an Ayushman card? No. It does not automatically confirm PM-JAY eligibility. Check on the official portal.
Is Ayushman Bharat enough for senior citizens? It gives people aged 70 and above cover of up to ₹5 lakh, but the same limits apply. If your parents may need major surgery, consider adding private cover.
